Second anniversary of Coast Guard helicopter crash

Friday, February 28 marks the two year anniversary of the deadly U.S. Coast Guard helicopter crash. The accident happened in 2012 during a nighttime training mission in Mobile Bay.

Lt. Cmdr. Dale Taylor, Lt. j.g. Thomas Cameron, Chief Petty Officer Fernando Jorge and Petty Officer 3rd Class Drew Knight of Thomasville were killed in the crash.

A report released in September 2013 stated pilot error as the primary cause of the accident. The report also listed other contributing factors, such as lack of established procedures for inadvertently entering instrument flight conditions. The investigation also concluded there was no misconduct involved in the crash.

A monument honoring the the crew members was dedicated March 2013 to the four who lost their lives. Coast Guard officials said private and community donations funded the monument.
